《收集個人資料聲明》
教會關懷貧窮網絡（統稱「本機構」）會盡力遵守《個人資料（私隱）條例》中所列載的規定，確保儲存的個人資料處理得到充份保護，及有妥善的儲存。為確保你能充分了解向本機構收集個人資料的準則，請細閱本聲明。
收集資料目的及使用準則；
1. 本機構可能使用你的個人資料（包括你的姓名、頭銜、電話號碼、傳真號碼、電郵、郵寄地址、所屬機構／公司／學校名稱等等），以便本機構日後與你通訊、處理報名、處理奉獻、發行收據和研究／分析／統計。
2. 在未得到你的同意時，本機構不會出售、租借或傳送你提供的個人資料給予其他機構、公司或持份者。
3. 向本機構要求提供個人資料純屬自願，如你未能提供足夠個人資料，本機構可能無法有效處理你的申請或提供有關服務。
4. 你可以隨時要求本機構停止使用你的個人資料作上述之用途，亦可以隨時要求更改你的個人資料。以上兩個安排無須繳付任何費用。
5. 當您閱覽本機構的網頁時，互聯網供應商可將您的資料紀錄下來，紀錄只會顯示您的網域名稱伺服器地址及所閱覽的網頁，而互聯網供應商亦只會向我們提供閱覽的總人數，這些資料只用於內部統計用途。
如你反對本機構將閣下的個人資料作上述之用途，請致電 3689 9810 或電郵至info@hkcnp.org.hk 通知本機構劉小姐。在未有收到你通知反對使用閣下的個人資料作上述的用途前，本機構將假設你不反對本處繼續使用閣下的個人資料作上述之用，直至本機構接獲你的另行通知為止。

《Personal Information Collection Statement》
Hong Kong Church Network for the Poor (HKCNP) undertakes to comply with the requirements of the Personal Data (Privacy) Ordinance to ensure that personal data are accurately and securely kept. To ensure you are well informed of the personal data as collected by HKCNP, please read through this guideline.
Purpose of Collection and Guideline for Use of Personal Data;
1. HKCNP may use your personal data in future (name, title, telephone number, fax number, email address, mailing address, name of belonged organisation/company/school etc.) for the purposes of contacting with you, handling applications, issuing receipts and research/analysis/statistical analyses.
2. Without your consent, HKCNP will not sell, lend or transfer your personal data to other organisations, companies or parties.
3. Providing personal data to HKCNP is on a voluntary basis. However, if you do not provide sufficient personal data, HKCNP may not be able to process your application or provide a certain service to you.
4. You have the right to request access to and correction of your personal data held by us free of charge.
5. When you visit HKCNP’s website, the service provider makes a record of your visit. This shows your Domain Name Server addressed and the pages you have visited. The service provider only discloses to us aggregate information regarding the number of visitors. HKCNOP will only use the information for statistical purposes.
If you deny HKCNP to use your personal data for the above purposes, please contact HKCNP Ms. Lau at 3689 9810 or email your request to info@hkcnp.org.hk . If HKCNP does not receive your disapproval for utilizing your personal data for the above purposes, HKCNP assumes that you agree to allow your personal data for the above purposes until your further notification.
